Valid service agreements

Creating a Service Agreement with an NDIS participant is best practice, and reduces the possibility of any misunderstandings, that all parties understand the support being provided and how much a participant will be charged during the life of a plan. Your client will have a limited budget that can not be exceeded. Everyone should be clear on all details related to the support, to protect the participant and the provider. Service Agreements are generally the best way to do this. As a provider, it is your responsibility to create a Service Agreement and Schedule as a means of discussing and deciding on the mutual agreement with your client.     


A valid Service Agreement should include, as a minimum:
◦  Details of the Provider and the Participant
◦  Service delivery dates (these must fall within the dates of the NDIS plan)
◦  Schedule of Supports with support description, costs (including any provider travel or transport costs), method of delivery, and total amount you intend to invoice for supports for the period of the agreement)
◦  Cancellation, Privacy and Dispute Resolution processes

Supplying a signed Service Agreement to the plan manager can support more efficient payment and enable us and the participant to track funds usage. In addition, we can quarantine the funds approved so that these are available to be drawn down when a valid invoice is received that reflects the support outlined.
